1.Application for Account Cancellation Notice

1.申請(qǐng)《注銷(xiāo)戶口通知》

(1)How to apply: you need to apply to the Exit and Entry Bureau of thePublic Security Bureau; Under the age of 16, the father (mother) mustapply to the Exit and Entry Administration Bureau of the PublicSecurity Bureau, and submit the birth certificate (or legal documentsto prove the relationship) and the identity document of the guardianand a copy; If the parents are divorced, the legal guardian of thechild must be present to apply, submitting the birth certificate (orlegal document proving the relationship), the child custody agreement(or legal document proving the child custody) and the guardian's IDcard and a copy.

(1)申請(qǐng)方式：需要由本人到公安局出入境管理局申請(qǐng)；未滿十六周歲的，須由父（母）到公安局出入境管理局申請(qǐng)，提交出生證（或證明親屬關(guān)系的法定文件）和監(jiān)護(hù)人的身份證件及復(fù)印件；如父母離異的，須由小孩的法定監(jiān)護(hù)人到場(chǎng)申請(qǐng)，提交出生證（或證明親屬關(guān)系的法定文件）、子女撫養(yǎng)權(quán)協(xié)議書(shū)（或證明子女撫養(yǎng)權(quán)的法定文件）和監(jiān)護(hù)人的身份證件及復(fù)印件。

2.The applicant's resident identity card (the original card iscollected);

3.在國(guó)外未入籍或無(wú)居留權(quán)，申請(qǐng)退出中國(guó)國(guó)籍，提供出入境管理部門(mén)簽發(fā)的《注銷(xiāo)戶口通知》（收原件）和公安部簽發(fā)的《中華人民共和國(guó)退籍證書(shū)》（核原件，收復(fù)印件）；

4.Have naturalized or have the right of residence abroad, provide the"Notice of Cancellation of Account" issued by the Exit andEntry Administration department (receive the original);

4.在國(guó)外已入籍或有居留權(quán)，提供出入境管理部門(mén)簽發(fā)的《注銷(xiāo)戶口通知》（收原件）；

6.以探親、旅游、商務(wù)、公務(wù)等非定居事宜出境前往香港、澳門(mén)地區(qū)，已取得當(dāng)?shù)赜谰镁恿魴?quán)，提供出入境管理部門(mén)簽發(fā)的《注銷(xiāo)戶口通知》（收原件）或《領(lǐng)取出境定居證件通知書(shū)》（核原件，收復(fù)印件）。
